We must receive your scholarship application and all required documentation before midnight on Sunday, March 2, 2025.

APPLICANT MUST BE AN ACTIVE MEMBER OF JOLT CREDIT UNION.

Mailed scholarship documents will not be accepted. You will be required to securely upload transcripts, tax records, and your personal essay at the bottom of this application.

PART 6 - PERSONAL ESSAY AND DOCUMENT UPLOAD

Documents must be uploaded via the secure links below to complete the application. YOU MUST READ AND COMPLY WITH ALL INSTRUCTIONS BEFORE SUBMITTING.

ESSAY:

Please upload an essay answering the question, “If you could change one thing in the world, what would it be?” Your essay must be between 500 and 600 words in length and must be submitted in .pdf or .doc format.

TRANSCRIPTS:

For school transcripts, official or unofficial records are acceptable. Transcripts may be submitted in .pdf or .jpeg format. Please do not submit files in .HEIC format.

TAX RECORDS:

Please upload the first two pages of your most recent tax return. This document will have the same Student AGI that you recorded in Part 4. If you do not work and do not have a tax return, please submit a 100-200 word document about how you account for your time spent not working. .pdf or .doc format is acceptable.

Please also upload the first two pages of your Parents’ most recent tax return. This document will have the same Parental AGI that you recorded in the section above. If you are independent (i.e., own your own home, have children, deceased parents, or receive limited help from your parents) submit your own tax documents twice.  

BOTH TAX FILES MUST BE UPLOADED FOR YOUR APPLICATION TO BE COMPLETE.

DO NOT SUBMIT: your FAFSA paperwork, your financial aid estimate, your school contribution estimate, your tax refund status, etc.

Tax records may be submitted in .pdf or .jpeg format. Please do not submit files in .HEIC format.

BEFORE UPLOADING:

Please rename all files before submitting, using the following format: LASTNAME, FIRST - (Essay, Transcript, Student Taxes, or Parent Taxes)

Prior to submitting via the secure link, you may mask any social security numbers.

If your application will not submit, please double check that your uploads are in place. You will be unable to submit your form without the required attached documents.

PLEASE NOTE – IF YOU DO NOT GET A POP UP SAYING YOUR APPLICATION HAS BEEN SUBMITTED, WE DID NOT RECEIVE YOUR APPLICATION.

If you receive an SQL error, hit the back button, check for any missing required fields. Double check the birthday fields and state drop down boxes, and that all your attachments are uploaded and then try submitting again.  After satisfying all required fields, you should be able to successfully submit your application and receive a confirmation.


PART 7 - SIGNATURE

I understand and agree to accept the decisions of the trustees at Jolt Credit Union as final and not open to contest. If I am awarded a scholarship, I hereby grant Jolt Credit Union permission to use my social security number for dispersal of funds. By submitting my application, I consent to receive email correspondence pertaining to the Monsignor Forbes Scholarship Trust Fund.
